Social Care Assessment and Services

If you have a disabled child whom you feel requires additional support, you are entitled to an assessment of his or her needs

The Family Contact Point (FCP) service aims to provide a rapid, effective and responsive screening service to all those seeking assistance for vulnerable children and families. This ensures that a child’s needs are identified early so that an appropriate care plan can be created as soon as possible.

In order to request an assessment, you can self-refer your child/family to social care by calling the following Family Contact Point number: Tel 01793 466903 (9.00am - 5.00pm)

Referrals can also be made by health and social care professionals, early years’ practitioners and anyone else involved with the child and his or her family, but these referrals should be made with the knowledge and agreement of the parents or young person where possible.

If the Assessment Team doesn’t feel that your situation meets the criteria for social support, they will signpost you to other relevant support services.

What Support is Available?

The types of support available include:





      • Emergency care
      • Behaviour Support
      • Direct Payments (to enable you to buy your own care)
      • Any additional support required to meet the assessed needs of your child
